The Veterans Services Case Manager offers services in a permanent supportive housing and affordable housing setting, integrating evidence-based practices effective with individuals who have experienced homelessness and have a history of chronic health, mental health, and/or co-occurring conditions and/or other socioeconomic barriers.  Such services include conducting initial assessments, developing individual goal plans, providing service coordination and case management, crisis intervention, increasing access to benefits/healthcare/mental healthcare, community integration, and community development within the apartment community.

The Veteran Services Case Manager ought to be familiar with implementing evidence-based practices such as: Housing First, Harm Reduction, Motivational Interviewing, Recovery Oriented Mental Health Services, Critical Time Intervention and Trauma-Informed Care. The employee will work collaboratively with a range of community service providers to coordinate and ensure tenants living in Archway Communities are stabilizing in housing, improving the quality of their lives, and integrating into the community.  Services will be delivered at the site, in the tenant’s homes, and in the community.


  • A. Supportive Services/Case Management
  • I. Objective:  Stabilize individuals and families in their housing communities.
  • II. Why this matters:  Archway wishes to create communities of dignity and respect for families, some of our residents require a greater level of support on an individualized basis.
  • III. Key Results:
    • a. Meet tenants at minimum 2x/month to provide outreach and engagement, strengths-based case management, and service coordination services designed to assist tenants in maintaining stable housing.
    • b. In collaboration with tenants, create a person-centered goal plan addressing short term and long-term goals to be reviewed and completed annually.
    • c. Provides in home and mobile case management/services coordination services in the areas of, but not limited to: independent living skills, housing stabilization, money management, community integration, employment, benefits establishment, linkage to community providers for problematic substance use, primary and mental health care, and all other services needed to assist tenants in reaching their treatment plan goals.
    • d. Conduct crisis and risk assessments in consultation with the supervisor/ team. Provide crisis intervention services focused on enhancing the tenants' ability to independently problem solve, utilize effective coping skills, and manage and self -coordinate their own care.
    • e. Use evidence-based practices in service delivery such as Motivational Interviewing, Harm Reduction, Critical Time Intervention and Housing First practices.
    • f. Provide onsite case management/services coordination services in the areas of independent living skills, community integration events, employment, benefits establishment, and other services as needed to assist tenants in reaching case plan and treatment plan goals.
    • g. Mediate and advocate on behalf of the tenant to help him/her maintain housing, health care benefits, entitlements, transportation, etc.
    • h. Promote tenant health and wellness through coordination services with local healthcare providers including, but not limited to: medical health, mental health, cognitive health, substance dependency, and trauma-services.
    • i. Create a monthly calendar of activities including programing at least 2x/week including 1 community meeting, 1 community integration outing, 1 guest speaker, 1 skill building class, 1 health-focused activity, Foodbank, Youth Programing/kids club.

         B. Documentation

  • I. Objective:  To provide a written record of events and services provided.
  • II. Why this matters:  Being able to communicate our Impact to Archway stakeholders is one of the most effective ways to tell our story and demonstrate our value.  In the context of demonstrating the services provided and their effectiveness “if it isn’t documented, it didn’t happen.”
  • III. Key Results:
  • a. Maintain documentation standards as set forth by the program contract and Archway Policies.
  • b. Complete progress notes on every face to face/ telephone contact with tenant or collateral contact within 48 hours.
  • c. Ensure tenant files are up to date with all file paperwork including consents, goal plans, and overall general documentation.
  • d. Submit all reports on/before due dates.
  • e. Input tenant data into online documentation systems within 48 hours.
  • f. Ensure data quality in online documentation system is up to date.
  • g. Plan and coordinate the on-site after-school, adult education, and community event programs.
  • h. Managing and supervising program participants, volunteers, teachers, providers, and instructors at the Community Center.
  • i. Implementing specific academic, enrichment, and recreational programs for youth participants in education, arts & crafts, physical fitness, healthy choices, and recreation.
  • j. Operate a minimum weekly Food Bank. Duties include picking up fresh produce (lifting 50 lb. boxes), setting up, distributing, and cleaning up food bank at minimum weekly.
  • k. Support with transportation needs, including ability to drive residents and children in a 15-passenger van to community outings and events.
  • l. Other duties as assigned that relate to the best interest of the Archway Communities.


  • A. Education and Experience:
    • a. At least one year of experience working with vulnerable individuals AND have a social work/mental health related bachelor's degree


    • b. At least two years of experience providing direct service coordination, mental health, and/or case management services.
  • B. Additional Experience Requirements:
    • a. Have experience working with clients with mental illness, chronic health issues, and/or substance use disorders.
    • b. Have expertise in the following areas: chronic homelessness, outreach and engagement strategies, intensive case management services, best practice models, mental health and substance use disorder services, crisis intervention, suicide assessment and prevention, affordable housing and public benefits applications, housing and landlord/tenant rights, eviction prevention, etc.
    • c. Applicants must have a Driver's License.
    • d. Preferred:
    • i. Bilingual language proficiency in addition to the primary language used in the workplace.
    • ii. Experience working with children and immigrants is preferred.
    • C. Personal Characteristics:
      • a. A high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs.
      • b. Ability to work independently and within a team environment and exercise mature judgment.
      • c. Ability to provide community and in-home based services on a regular basis.
      • d. Strong written and verbal communication skills.
      • e. High level of motivation and self-starting nature with the ability to coordinate multiple projects/tasks simultaneously in a fast-paced environment.
      • f. Ability to maintain and execute confidential information according to HIPAA standards.
      • g. Collaborative and oriented toward team success
    • a. Ability to efficiently perform job responsibilities with minimal supervision.
    • b. Follow Archway’s COVID protocols.
    • c. Employee may be required to provide coverage on weekends, evenings, and holidays as needed.
    • d. The employee may be in contact with individuals and families in crisis who may be ill, using alcohol and drugs, and who may not be attentive to basic personal hygiene, health and safety practices. The employee may experience a number of unpleasant sensory demands associated with the client’s use of alcohol and drugs, and lack of personal hygiene.
    • e. The employee is a self-starter and must be ready to respond quickly and effectively to many types of situations, including crisis situations and potentially hostile situations.
    • f. The noise level in the work environment is usually moderate in an office setting. Sometimes work may become stressful when working under pressure.
    • g. The employee can provide structure and firm, appropriate boundaries for youth between the ages of 5-18 years of age and able to provide outreach services to resident families and assist in possible conflict resolution.
    • h. The employee must have the ability to relate to and excel within a diverse cultural community.
    • a. The pay range for this role begins at $22 per hour and goes to $24 per hour depending on experience.
    • b. Archway provides Health and Dental insurance with the company paying 80% of the cost.
    • c. Archway provides a Vision plan, the company pays the full amount of this coverage.
    • d. Archway provides a 401k retirement plan.  We currently do not provide matching funds.
    • e. Archway provides 12 paid holidays per year, including 3 floating holidays.
    • f. Full-time employees accumulate one day of paid sick leave for each month worked, after their first 30 days of full employment. Unused sick leave accumulates from year to year up to a maximum of 60 working days.
    • g. Full-time employees in their first three years of continuous employment will accrue vacation time at 3.08 hours per pay period.  For a Full-Time employee working 40 hours per week, this equals 80 hours, or two-weeks of vacation per year.
    • h. Prior to hire, an Employee Handbook detailing these and other benefits will be made available.
